About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve El Dorado Hills and the surrounding areas. As an independent contractor, you are your own boss and control your own schedule. FeldCare Connects provides administrative support including assigning, communication, scheduling, care coordination, and quality assurance. Clinicians provide one-on-one treatment, determine the plan of care, and use an intuitive Electronic Medical Record system for documentation. The company connects clinicians to patients in their community by contracting with home health agencies, allowing them to provide necessary care to homebound patients.

Requirements

Current Physical Therapy license issued by the state board of Physical Therapy.
The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
Completion of an accredited Physical Therapist program.
Reliable personal transportation.
Smartphone.

Responsibilities

Deliver quality home healthcare to patients of all ages.
Observe, record, and report the patient's response to treatment and changes to the patient's condition.
Determine the plan of care.
