About The Position

The Physical Therapist provides evidence-based outpatient rehabilitation services to patients with musculoskeletal, movement, mobility, and functional impairments. The Physical Therapist independently evaluates patients, establishes measurable functional goals and individualized plans of care, and provides and progresses skilled interventions based on clinical findings, patient response, standardized outcomes, and continued medical necessity. The Physical Therapist manages an assigned outpatient caseload; provides patient and caregiver education and individualized home programs; monitors functional progress; and coordinates appropriate referral, transition, or discharge. The role collaborates with physicians, providers, and other members of the care team to support continuity of care and promptly communicates significant findings, changes in condition, or barriers to progress. The Physical Therapist maintains accurate and timely documentation, supports appropriate coding and charge capture as applicable, and meets established department expectations for access, productivity, documentation timeliness, and efficient utilization of scheduled clinical time while maintaining clinical quality and patient safety. The Physical Therapist practices within California licensure requirements, applicable professional standards, and MarinHealth policies and demonstrates MarinHealth's C.A.R.E.S. values.

Requirements

Graduate of an accredited physical therapy program required.
Degree and educational preparation must support eligibility for California Physical Therapist licensure.
Demonstrated education, clinical training, or professional experience in physical therapy evaluation, treatment planning, therapeutic intervention, reassessment, patient education, and discharge planning.
Demonstrated ability to establish measurable functional goals, evaluate treatment effectiveness, and modify the plan of care based on objective findings and patient response.
Ability to independently organize and manage an outpatient caseload while maintaining timely documentation and safe, patient-centered care.
Physical Therapist (PT) required at hire.
Basic Life Support (BLS) required at hire.
Integrative Agitation Management (IAM) required within 30 days of hire.
Maintains all licenses, certifications, and continuing education required for practice throughout employment.
Demonstrates clinical judgment and competency in physical therapy evaluation, assessment, goal setting, treatment planning, intervention, reassessment, and discharge planning consistent with current standards of practice.
Organizes and prioritizes patient care responsibilities effectively in a changing clinical environment.
Completes accurate, timely, and clinically relevant documentation in accordance with MarinHealth policy, regulatory requirements, payer requirements, and professional standards.
Communicates clearly and professionally with patients, families/caregivers, physicians, colleagues, and members of the interdisciplinary care team.
Provides effective patient and caregiver education, including home programs, precautions, equipment use, and strategies that support safe continuity of care.
Demonstrates competency using the electronic health record and other technology required for the role.
Uses sound judgment to recognize changes in patient condition, modify care within scope, and escalate concerns appropriately.
Demonstrates reliability, professional accountability, adaptability, and commitment to evidence-based practice and continuous learning.

Nice To Haves

Prior outpatient physical therapy experience, particularly in an orthopedic or musculoskeletal setting.
Experience with musculoskeletal examination, therapeutic exercise, neuromuscular re-education, functional training, manual interventions, and individualized home exercise programs.
Experience using standardized functional outcome measures to establish baseline status, monitor progress, and support clinical decision-making.
Experience collaborating with referring providers and other healthcare professionals regarding patient progress, changes in condition, additional evaluation needs, and discharge planning.
Experience working within an electronic health record and outpatient environment requiring timely documentation, appropriate charge capture, and efficient management of scheduled patient care.

Responsibilities

Documents evaluations, treatment, progress, communication, and discharge information accurately and within required timeframes in the electronic health record.
Collaborates with physicians/providers and the interdisciplinary care team regarding patient status, functional needs, precautions, equipment, discharge planning, and changes requiring additional clinical attention.
Delegates and supervises physical therapy services provided by Physical Therapist Assistants and support personnel in accordance with California scope-of-practice requirements, MarinHealth policy, and demonstrated competency.
Maintains accountability for delegated care and required documentation.
Maintains responsibility for the physical therapy plan of care, including evaluation, clinical decision-making, treatment progression, reassessment, and discharge consistent with applicable scope and regulatory requirements.
Recognizes changes in patient condition or safety risk and initiates appropriate intervention, escalation, referral, or communication within scope and MarinHealth policy.
Uses equipment and resources safely, appropriately, and responsibly; reports equipment, environmental, or patient-safety concerns promptly.
Participates in department quality, safety, patient-experience, and performance-improvement activities and applies evidence-based practices to support clinical outcomes.
Maintains required competencies, licensure, certifications, and professional knowledge; incorporates current standards and evidence into practice.

Establishes measurable, patient-centered goals and develops an individualized plan of care based on evaluation findings, patient priorities, clinical judgment, applicable physician/provider orders, and current standards of practice.
Provides skilled physical therapy interventions within scope of practice and adjusts treatment based on patient response, progress, safety, and clinical findings.
Reassesses patients at clinically appropriate intervals, measures progress toward established goals, modifies the plan of care as indicated, and completes appropriate discharge planning.
Educates patients and families/caregivers regarding diagnosis-related functional needs, treatment plans, home programs, precautions, equipment, self-management, and strategies to support safe continuity of care.
Recommends and, when authorized, assists with implementation of new clinical methods, workflows, education, or process improvements.
Supports department operations such as scheduling coordination, supply/equipment stewardship, charge capture, and other administrative activities as assigned.
Serves as a clinical resource or provides coverage for designated department responsibilities when assigned and within demonstrated competency and scope.
Performs other duties as assigned that are consistent with the role's scope, qualifications, and operational needs.
