The Physical Therapist provides evidence-based outpatient rehabilitation services to patients with musculoskeletal, movement, mobility, and functional impairments. The Physical Therapist independently evaluates patients, establishes measurable functional goals and individualized plans of care, and provides and progresses skilled interventions based on clinical findings, patient response, standardized outcomes, and continued medical necessity. The Physical Therapist manages an assigned outpatient caseload; provides patient and caregiver education and individualized home programs; monitors functional progress; and coordinates appropriate referral, transition, or discharge. The role collaborates with physicians, providers, and other members of the care team to support continuity of care and promptly communicates significant findings, changes in condition, or barriers to progress. The Physical Therapist maintains accurate and timely documentation, supports appropriate coding and charge capture as applicable, and meets established department expectations for access, productivity, documentation timeliness, and efficient utilization of scheduled clinical time while maintaining clinical quality and patient safety. The Physical Therapist practices within California licensure requirements, applicable professional standards, and MarinHealth policies and demonstrates MarinHealth's C.A.R.E.S. values.
