The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement. This role provides physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act. The position involves assessing and evaluating therapeutic/rehabilitative/functional status, developing the total plan of care, and evaluating the home environment. It also includes assessing muscle strength, mobility, gait, ROM, and potential for rehab, directing physical therapy treatment, and instructing patients and families/caregivers on the use and care of therapeutic appliances. The therapist determines priority needs for physical therapy, reports patient reactions to treatment or changes in condition to the physician, and initiates physical therapy programs. They also instruct other personnel and/or family/caregiver members in certain phases of physical therapy and the goals of the program. Training patients in the use of prosthetic devices may also be required. The role involves identifying patient and family/caregiver needs for other home health services and making referrals as necessary. Preparation and submission of clinical and progress summaries based on goal attainment, participation in discharge planning, and providing physical therapy consultation are key responsibilities. The therapist will also provide in-service education programs for nursing organization personnel and participate in peer consultation. Supervision of physical therapy assistants according to organization policy and state regulations is required. When therapy is the only skilled service, the therapist instructs, supervises, and evaluates home health aide care every two (2) weeks. Adherence to company attendance and reliability guidelines is essential for ensuring quality and consistent patient care.
