About The Position
Human Touch Home Health is seeking a licensed Physical Therapist (PT) to join their team in Washington, DC. This is a full-time opportunity offering flexibility, autonomy, and the chance to build meaningful relationships with patients. As a home health Physical Therapist, you will work one-on-one with patients in their homes to help them regain strength, mobility, and independence. You will have the freedom to manage your schedule while focusing on delivering exceptional care.
Requirements
- Active Washington, DC Physical Therapist license required
- Valid driver’s license and reliable transportation
- Strong documentation and communication skills

- Home health experience preferred
Responsibilities
- Complete evaluations, reassessments, and discharge planning
- Develop individualized treatment plans
- Provide skilled physical therapy services in patients’ homes
- Educate patients and caregivers on exercise programs and safety techniques
- Collaborate with physicians and the interdisciplinary team
- Complete documentation accurately and on time
