About The Position

Innovate Rehab and Wellness is seeking a Licensed Physical Therapist (PT) for a Full-Time position in a Skilled Nursing Facility in Winnfield, LA. This role focuses on providing one-on-one patient care with a flexible Monday-Friday schedule, emphasizing clinical outcomes and patient satisfaction. Requirements

• Doctorate or Master’s degree in Physical Therapy (accredited program)
• Valid state PT license
• Strong communication, problem-solving, and patient-care skills
• CPR certification preferred

Nice To Haves

• Clinical experience preferred; new grads welcome

Responsibilities

• Perform comprehensive PT evaluations
• Develop personalized, evidence-based treatment plans
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training
• Educate patients and caregivers on home programs and injury prevention
• Track and document progress; adjust plans as needed
• Collaborate with physicians, nurses, OTs, and other clinicians
• Stay current with PT best practices through continuing education
• Ensure compliance with all regulatory and ethical standards
