About The Position

The Physical Therapist in the skilled nursing and long term care setting focus on restoring elder functional mobility, preventing elder physical decline, and assist in managing elder chronic pain. The Physical Therapist evaluates elders and conducts medically prescribed physical therapy treatment programs to improve elder functional status. They provide education to elders in regard to injury prevention, ergonomics and rehab therapy. They are an essential member of a collaborative team.

Requirements

  • Current state of Michigan Physical Therapist license
  • Bachelor’s degree
  • State of Michigan PT License required at hire or within two months of hire date.
  • Basic Life Support certification is required

Nice To Haves

  • Minimum of 2 years’ experience in outpatient services preferred
  • Master’s and/or Doctoral degree preferred

Responsibilities

  • Performs initial and ongoing assessments/evals of the elder’s condition and provides appropriate treatment. This includes Med A, Med B, and change in condition
  • Sees all Med A skilled elders accordingly
  • Evaluates elders for Med B services and sees accordingly
  • Assists with range of motion exercises, ambulation, and supports elders in regaining mobility and strength
  • Observes and documents elder progress, reporting any changes or concerns to the healthcare team
  • Offers encouragement and support to elders throughout their health journey
  • Ensures the elder’s safety during all activities and reports any potential hazards
  • Encourages the elder to perform tasks for him/herself as appropriate and as assigned in the elder’s care plan
  • Collaborates with nurses, therapists, and other team members to ensure coordinated and effective care and notifies them of any problems, referrals or reassessments that are needed
  • Reports to the nurse any adverse conditions experienced by the elder during the therapy session including, but not limited to, pain
  • Accurately records and documents elder progress and interventions
  • Utilizes equipment and devices appropriately and in a safe manner. Monitors the condition of equipment and devices arranging for repair or replacement of equipment as needed
  • Assists in equipment management and inventory
  • Adheres to established safety and infection control protocols
  • Maintains elder confidentiality
  • Acts as a vital member of the healthcare team, dedicated to helping patients recover and regain their independence through personalized care and support
  • Operates within the guidelines consistent with accepted/legal standards of practice relevant to an individual’s level of training and education
  • Attends weekly IDT meeting and provides update on elder status
  • Completes MDS therapy and GG sections as required
  • Assesses elders for mobility and transfers upon admission and as needed upon change in condition
  • Determines the appropriate mechanical lift and sling size for elder. Reassesses sling fit every 3 months.
  • Educates, coaches, and mentors frontline staff regarding appropriate lift and equipment use, updates/changes to elder’s care plan.
  • Works collaboratively with Restorative Therapy to grow the program. Covers Restorative Therapy sessions in the absence of the PTA.
  • Establishes a plan of care which is appropriate to problems identified and involves the elder and/or DPOA according to the elder’s capabilities and desires.
  • Plans, prepares and carries out individually designed programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in elders.
  • Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the age and condition of the elder.
  • Develops and maintains rapport with all staff and providers to ensure the best possible outcome for the elder’s care and treatment.
  • Administers treatment involving application of physical agents, using equipment, and ultrasound machines.
  • Evaluates, fits, and adjust prosthetic and orthotic devices and recommends modifications.
  • Identifies and documents goals, anticipated progress and plans for reevaluation.
  • Performs accurate documentation of all care rendered.
  • Incorporates the Eden Philosophy of living into every action undertaken throughout one’s employment.
  • The person holding this position is delegated the responsibility for carrying out the assigned duties and responsibilities in accordance with current existing federal and state regulations and established company policies and procedures
  • Performs other job duties as assigned.
