Requirements
- Bachelor’s in Physical Therapy from an accredited program
- Active State of Ohio Physical Therapist License
- CPR certified
Responsibilities
- Evaluate & treat patients in accordance w/state & national practice acts.
- In cooperation w/referral source, prov therapy evaluation to determine extent of symptoms &/or functional limitations relevant to diagnosis.
- Develop treatment plan that is targeted & adjusted to decrease or eliminate symptoms & functional limitations.
- Direct patient care, day to day patient scheduling, supervision of Physical Therapist Assts & students, consultation w/physicians & other team members, documentation in elec medical record, QA & performance improvement (QAPI), clinical educ, guest relations & billing for patient services rendered &other duties as assigned.
