About The Position
Seeking a compassionate, motivated Physical Therapist (PT) to provide exceptional care in a collaborative post-acute/skilled nursing setting in Traverse City, Michigan. This role is suitable for both experienced clinicians and new graduates, offering mentorship, career growth, and support. The position focuses on making a difference every day by valuing expertise and prioritizing patients.
Requirements
- Graduate of an accredited Physical Therapy program
- Current Michigan Physical Therapist license (or eligible for licensure)
- Strong communication and clinical skills
Nice To Haves
- Experience in Skilled Nursing, Post-Acute Care, Inpatient Rehab, Home Health, or Outpatient is preferred but not required
- New graduates are encouraged to apply
Responsibilities
- Evaluate and develop individualized treatment plans
- Provide evidence-based physical therapy services to adult and geriatric patients
- Help patients regain strength, mobility, balance, and independence
- Collaborate with PTAs, OTs, OTAs, SLPs, nursing, and physicians
- Complete timely documentation in accordance with Medicare and regulatory guidelines
- Participate in interdisciplinary care planning and discharge planning
- Deliver outstanding patient-centered care while achieving positive clinical outcomes
