About The Position

Innovate Rehab and Wellness is seeking a Licensed Physical Therapist (PT) for a Full-Time position in a Skilled Nursing Facility in Natchitoches, LA. The schedule is Monday – Friday with flexibility. We are a therapist- and veteran-owned company with over 25 years of experience providing industry-leading, patient-centered care. We offer a supportive, collaborative clinical team and a focus on one-on-one patient care, with group/concurrent treatments under 1%. Requirements

Doctorate or Master’s degree in Physical Therapy (accredited program)
Valid state PT license
Strong communication, problem-solving, and patient-care skills
CPR certification preferred

Nice To Haves

Clinical experience preferred; new grads welcome

Responsibilities

Evaluate patients and identify movement and functional limitations
Create and lead individualized treatment plans
Help restore movement, function, and quality of life
Work closely with an interdisciplinary team for optimal recovery
Perform comprehensive PT evaluations
Develop personalized, evidence-based treatment plans
Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training
Educate patients and caregivers on home programs and injury prevention
Track and document progress; adjust plans as needed
Collaborate with physicians, nurses, OTs, and other clinicians
Stay current with PT best practices through continuing education
Ensure compliance with all regulatory and ethical standards
