About The Position

Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, is seeking a full-time Physical Therapist (PT) to join their award-winning team. This role involves performing patient assessments, developing treatment plans, collaborating with a care team, and educating patients and families. Requirements

  • Must have a current PT license (state-specific).
  • Graduate of an accredited physical therapy program.
  • Valid BLS certification.
  • Minimum one year of PT experience (home health experience preferred).
  • Excellent communication, organizational, and time management skills.
  • Must have a valid driver’s license and access to a reliable and insured vehicle.
  • Ability to visit homes that may not be wheelchair accessible.
  • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
  • Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.

Nice To Haves

  • Proficiency with electronic medical records is a plus.

Responsibilities

  • Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
  • Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.
  • Collaborate with the interdisciplinary care team to optimize patient outcomes.
  • Provide education and support to patients and families/caregivers on therapy programs.
  • Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
  • Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
  • Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
  • Identify patient and family/caregiver needs for additional services and make appropriate referrals.
  • Confirm daily visit schedules to ensure seamless coordination with other staff.
  • Communicate patient progress and changes with physicians and care teams.
  • Document all visits and progress notes on the same day of service.
