About The Position

This is a chance to practice physical therapy the way it was meant to be practiced: one patient at a time, in the setting where recovery actually happens — their own home. As a Home Health Physical Therapist, you'll evaluate and treat patients under physician orders, using targeted therapeutic techniques to restore strength, coordination, and mobility. You'll work directly with patients recovering in the comfort of their own homes, giving you the time and space to focus on what matters most: their progress. You'll work as part of an interdisciplinary care team, contributing your clinical expertise to a shared plan of care rather than working in isolation. Home care experience is valued here, and if you have it, this role lets you keep doing the work you already know how to do well — with patients who need focused, individualized attention. You'll have the independence to manage your own field schedule and clinical decision-making, while staying connected to a broader care team for support.

Requirements

  • Graduate of an accredited Physical Therapy program
  • Current, active Physical Therapist license in California
  • CPR certification
  • Negative TB test and any additional screenings required by agency policy
  • Home care experience
  • Valid driver's license and auto insurance
  • Ability to perform field work and lift a minimum of 25 lbs
  • Comfortable working independently as well as collaboratively with a care team

Responsibilities

  • Travel to patients' homes to conduct evaluations and treatment sessions
  • Design and carry out therapeutic interventions to address muscle strength, coordination, body alignment, pain, and functional movement
  • Collaborate with physicians and the interdisciplinary team to align treatment with each patient's care plan
  • Work independently in the field while communicating regularly with team members
