About The Position

Adoration Home Health is seeking a dedicated Home Health Physical Therapist to provide expert, patient-centered care in Salisbury, Maryland. The role offers one-to-one patient care, flexible scheduling options, independent work with team support, and opportunities for advancement. The position covers Salisbury, MD, and is full-time. A $10,000 sign-on bonus is available to external candidates only.

The Physical Therapist evaluates and treats patients according to physician orders, establishes measurable goals, and develops treatment plans to help patients achieve their highest possible level of independence and function. The therapist collaborates with the treatment team, physicians, patients, and families; maintains timely service documentation; and supervises Physical Therapist Assistants and Physical Therapy Aides in accordance with state practice requirements.

Requirements

• Graduate of a Physical Therapist education program approved by CAPTE.
• Current Physical Therapist licensure in the state of practice.
• American Heart Association CPR certification; CPR certification may be offered and completed upon hire.
• Two years of experience in adult physical therapy.
• Excellent observation, verbal and written communication, problem-solving, and basic math skills.
• Physical therapy skills as outlined in the competency checklist.

Nice To Haves

• Home health experience is preferred.

Responsibilities

• Perform physical therapy evaluations and treatments based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and function.
• Maintain communication among the treatment team, physician, patient, and family members, as applicable.
• Maintain thorough and timely documentation of services provided to ensure proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with the state practice act.
