The Physical Therapist (PT) is responsible for evaluating and assessing the needs of referred patients in a home care environment, developing treatment plans, and providing therapy services in collaboration with physicians and case managers. This role encourages creativity and latitude in performing various tasks.
Requirements
- Graduate of a Baccalaureate, Master's or Doctorate program in Physical Therapy program accredited by a Physical Therapy accreditation.
- Current and unencumbered license to practice as a Physical Therapist within the state of employment.
- Two (2) years of appropriate experience as a Physical Therapist.
- Demonstrates good verbal and written communication and organization.
- Possess and maintain valid CPR (must have obtained in-person with a practical skills evaluation as part of the final test).
- Access to reliable transportation.

- Community/home health experience is preferred.
