About The Position

At Sunnyvale Gardens Post-Acute, we are dedicated to providing outstanding short-term rehabilitation and long-term care. Our team is passionate about helping residents regain their independence, strength, and quality of life in a supportive, welcoming environment. We pride ourselves on a collaborative culture where clinical excellence and patient-centered care come first. We are seeking a compassionate, skilled, and motivated Physical Therapist (PT) to join our rehabilitation team full-time. In this role, you will evaluate, plan, and administer personalized physical therapy programs to help our residents achieve their maximum functional mobility. You will work closely with a multidisciplinary team of doctors, nurses, and fellow therapists to deliver top-tier post-acute care.

Requirements

Master’s or Doctorate degree in Physical Therapy from an accredited program.
Active, unrestricted Physical Therapist license in the State of California.
Current CPR/BLS certification.
Strong communication, interpersonal, and time-management skills.

Nice To Haves

Experience in a skilled nursing facility (SNF), post-acute care, or outpatient rehab setting is preferred, but passionate new graduates are welcome to apply!

Responsibilities

Conduct comprehensive initial evaluations of residents to determine their physical condition, limitations, and rehabilitation potential.
Develop, implement, and modify individualized plans of care based on resident needs and physician orders.
Provide high-quality evidence-based physical therapy treatments (including strength training, balance exercises, and mobility training).
Accurately document resident progress, treatments, and discharge plans in compliance with federal, state, and facility guidelines.
Educate residents and their families on post-discharge exercises, safety protocols, and assistive devices.
Collaborate effectively with the interdisciplinary care team to optimize patient outcomes.
