About The Position
Always Best Care is seeking a licensed Physical Therapist to provide skilled home health services throughout O‘ahu. You will evaluate patients, develop treatment plans, and provide therapy interventions designed to improve mobility, safety, strength, and overall function. Always Best Care is a Medicare-certified Home Health agency serving patients throughout O‘ahu. We focus on helping seniors maintain independence and quality of life through skilled home health services delivered in the comfort of their homes.
Requirements
- Current Hawaii Physical Therapist license
- Minimum one year of clinical experience
- Strong communication and documentation skills
- Ability to work independently
- Valid driver's license and reliable transportation
Nice To Haves
- Home health experience preferred
Responsibilities
- Provide skilled physical therapy services in patients' homes
- Complete evaluations, reassessments, and fall risk assessments
- Develop individualized treatment plans
- Improve patient mobility, balance, strength, and safety
- Educate patients and caregivers on exercises and fall prevention
- Communicate patient progress with the interdisciplinary care team
- Maintain Medicare-compliant documentation
