About The Position

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) to join our new Central New Jersey Senior Living team, a Medicare-certified home health office. In this role, you'll provide one-on-one physical therapy services to adult and geriatric clients living in assisted living, independent living, senior congregate living, 55+ communities, and intermediate care facilities serving individuals with intellectual disabilities throughout Somerset, Middlesex, Monmouth, and Union Counties, NJ. This position may also include visits in traditional home health community settings. As a valued member of our interdisciplinary care team, you'll help clients improve mobility, maximize independence, and safely remain in the place they call home.

Requirements

  • Current New Jersey Physical Therapist (PT) license.
  • Minimum of one year of recent experience as a Physical Therapist.
  • Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) or equivalent accrediting body.
  • Ability to work independently and effectively manage time.
  • Strong communication, organizational, and clinical assessment skills.

Nice To Haves

  • Experience with electronic medical records (EMR) preferred.

Responsibilities

  • Perform skilled physical therapy visits throughout designated territories.
  • Complete comprehensive physical therapy evaluations and ongoing reassessments.
  • Assess mobility, strength, balance, gait, transfers, endurance, range of motion, and overall functional status.
  • Develop and implement individualized treatment plans using evidence-based interventions, including therapeutic exercise, gait training, balance training, and manual therapy techniques.
  • Implement skin integrity interventions to help prevent wound development and promote healing when appropriate.
  • Collaborate with nurses and interdisciplinary team members to coordinate exceptional patient care.
  • Educate clients, families, and caregivers on therapeutic exercises, safety strategies, mobility techniques, and disease management.
  • Complete timely and accurate documentation using BAYADA's electronic medical record (EMR) system.
