As an AgeRight Home Health PT, you’ll be delivering person-centered care to grateful and highly motivated patients, many of them seniors, helping them achieve therapy goals and a higher quality of life in their time of greatest need. Requirements
- Must be licensed as a Physical Therapist in the state of Oregon
- Graduate from an accredited program with a degree in Physical Therapy
- CPR Certification
- National Provider Identifier (NPI) required upon hire

- Organizational skills to facilitate program development
- Strives to positively represent their discipline and AgeRight Home Care in the community
Responsibilities
- Complete assessments for every patient upon admission
- Establish treatment plans
- Teaches patient and family/caregivers in meeting individual treatment goals, and treatment plan implementation
