About The Position

Visalia Post Acute is seeking a skilled and compassionate Physical Therapist (PT) to join our rehabilitation team. This full-time opportunity is ideal for a PT who is passionate about restoring function, mobility, and independence for residents in a skilled nursing and rehabilitation setting. The Physical Therapist (PT) evaluates and treats residents, develops and implements individualized treatment plans, collaborates with physicians and interdisciplinary team members, and maintains accurate and timely documentation in the medical record. The PT also provides clinical leadership and education to support optimal resident outcomes.

Requirements

  • Bachelor’s degree in Physical Therapy required
  • Current, active California Physical Therapist license in good standing
  • Must meet all continuing education requirements per state regulations
  • Proficient in electronic documentation systems (Casamba and PointClickCare preferred)
  • Strong clinical judgment, communication, and organizational skills
  • Ability to work independently and collaboratively within a team environment
  • Frequent standing, walking, pushing, pulling, and reaching
  • Ability to lift and move up to 50 pounds frequently
  • Occasional stooping, kneeling, crouching, and balancing
  • Visual and auditory acuity sufficient for patient care and documentation

Nice To Haves

  • Master’s or Doctorate degree in Physical Therapy preferred
  • Skilled nursing or geriatric experience preferred but not required

Responsibilities

  • Evaluate residents within 24 hours (or one business day) of physician referral; document delays appropriately
  • Develop individualized treatment plans and obtain physician approval for services
  • Provide physical therapy treatment per the physician-approved plan of care
  • Supervise Physical Therapy Assistants (PTAs), aides, and students in accordance with the state practice act
  • Communicate resident progress, challenges, and care plans with the rehab and interdisciplinary teams
  • Participate in patient care conferences, utilization review meetings, and rehab meetings as needed
  • Complete evaluations, daily treatment notes, weekly progress notes, recertifications, and discharge documentation per regulatory and payer requirements
  • Educate residents, families, and caregivers on mobility, safety, exercises, and discharge planning
  • Participate in discharge planning and coordination of care
  • Recommend and facilitate ordering of durable medical equipment (DME) to promote safe mobility and independence
  • Conduct in-services and training for facility staff as assigned
  • Report equipment issues promptly to ensure safe and effective operation
  • Maintain positive, professional relationships with residents, families, and staff
