About The Position

St. Elizabeth, a Vierra Communities property, is seeking a Physical Therapist to join their dynamic team in a newly renovated, bright, state-of-the-art therapy suite. The facility features a bright and airy open-concept therapy floor designed for both patient healing and provider well-being. The role involves optimizing the physical abilities and skills of patients, initiating referrals, evaluating and treating patients, complying with Minimum Data Set procedures, participating in care planning, and documenting progress. The ideal candidate is motivated, reliable, has a strong work ethic, and is committed to providing compassionate and excellent care.

Requirements

  • License to practice Physical Therapy within the state
  • Successful completion of the National Certification Examination for Physical Therapist
  • Must pass a criminal background check
  • Possess strong communication and interpersonal skills
  • Must have the patience, empathy, and compassion needed to care for elderly patients
  • Ability to work cooperatively with other healthcare professionals

Nice To Haves

  • Experience with long-term care and /or geriatric care is preferred

Responsibilities

  • Optimize the physical abilities and skills of patients by performing or supervising therapy staff in initiating referrals, evaluating and treating appropriate patients, complying with Minimum Data Set procedures, effectively participating in the care planning process, appropriately documenting progress toward identified objectives and communicating patient status and needs to the patient, the patient’s family, the staff and other professionals as appropriate.
  • Conduct screenings of patients at regular intervals to determine need for intervention/treatment.
  • Establish individual patient schedules in conjunction with other team members.
  • Develop treatment plans by identifying a problem list, including long and short-term goals and methods to achieve identified goals.
  • Coordinate treatment plan with patient, family and other team members.
  • Implement and conduct treatment as outlined in the treatment plan.
  • Monitor patient’s response to intervention and modify treatment as indicated to attain goals in accordance with physicians’ orders.
  • Develop appropriate home or community programming to maintain and enhance the performance of the patient in his/her own environment.
  • Document result of patient’s assessment, treatment, follow-up and termination of services.
  • Adapt treatment to meet the individual needs of the patients.
  • Provide/recommend/fabricate adaptive devices, orthotics or prosthetics. Train patient, family and appropriate staff in the use or application of the above.
  • Participate in discharge planning.
  • Perform other duties or special projects as assigned and per JD.
  • Must be able to write, read and speak English.
