About The Position
Health Link Home Health is seeking a Physical Therapist (PT) to join their team. This role focuses on providing patient care in a home health setting, with a unique support model designed to minimize administrative burdens for therapists. The company emphasizes allowing Physical Therapists to focus on patient care, supported by dedicated teams for scheduling, intake, authorization, and clinical coordination, as well as AI-assisted documentation technology. This approach aims to reduce charting time and administrative tasks, enabling therapists to spend more time treating patients.
Requirements
- Active California Physical Therapist License
- Valid driver's license
- Strong communication and documentation skills

- Home health experience preferred, but not required.
Responsibilities
- Perform physical therapy admissions, evaluations and reassessments
- Develop individualized treatment plans
- Provide skilled physical therapy services in the home setting
- Supervise Physical Therapist Assistants (PTAs)
- Improve patient mobility, strength, balance, safety, and independence
- Educate patients and caregivers
- Collaborate with physicians and interdisciplinary team members
- Complete documentation accurately and timely
