About The Position

Brighten Life Home Health is hiring a Physical Therapist to provide personalized home health care to patients in Blanding, Utah. The role involves one-on-one patient care, supporting patients as they recover, improve mobility, and maintain independence in their homes. The therapist works with patients, families, and an interdisciplinary healthcare team and has autonomy in patient care and treatment planning. The position is offered part-time and PRN, with flexible scheduling. Mileage reimbursement is $0.56 per mile. Pay is based on experience, qualifications, and skills.

Requirements

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the State of Utah.
• Strong clinical, assessment, and treatment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a healthcare team.
• Strong organizational and documentation skills.

Responsibilities

• Evaluate patients and participate in therapy screenings.
• Develop and implement individualized treatment plans.
• Provide physical therapy services to improve mobility, strength, and function.
• Collaborate with patients, families, and the healthcare team to achieve therapy goals.
• Educate patients and caregivers on treatment plans and home exercise programs.
• Maintain timely and accurate documentation.
