Responsibilities
- Evaluate patients and build individualized care plans.
- Adjust care plans as progress unfolds.
- Perform skilled therapy in patients' homes.
- Communicate patient progress and changes to physicians.
- Guide patients and families through home exercise programs.
- Supervise PTAs and HHAs as needed.
- Complete documentation, including OASIS, progress notes, and discharge summaries.
- Apply sound clinical judgment to every visit.
