About The Position

FOX Rehabilitation is expanding in Richardson, TX, and is seeking dedicated, licensed Physical Therapists to provide exceptional one-on-one care to older adults in their homes or within Senior Living Communities. Utilizing the Geriatric House Calls™ model, you will help patients enhance their mobility, independence, and overall quality of life. This role is perfect for Physical Therapists who value autonomy, flexibility, and achieving meaningful patient outcomes, with the ability to manage their own treatment schedules and foster strong patient relationships. The role may also incorporate telehealth and virtual care options, offering increased flexibility, improved care continuity, and the capacity to serve more patients. Requirements

  • Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
  • Degree from an accredited Physical Therapy program
  • Basic computer and EMR proficiency
  • Current CPR certification

Responsibilities

  • Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
  • Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
  • Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
  • Document effectively within FOX’s EMR system in accordance with regulatory standards
  • Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
  • Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
  • Provide telehealth / virtual care when appropriate to extend access and flexibility
