About The Position

This is a permanent position for a Physical Therapist at a Continuing Care Retirement Community (CCRC) with over 200 units, offering Skilled Nursing Facility (SNF) and Assisted Living/Independent Living (AL/IL) services on the same campus. The role involves assessing patients' physical condition and medical history, developing customized treatment plans, implementing therapeutic techniques to improve mobility and strength, and educating patients on home care. The Physical Therapist will monitor patient progress, collaborate with other healthcare professionals, maintain accurate records, and stay updated on the latest advancements in physical therapy. Responsibilities also include providing pain management techniques and guiding patients in their rehabilitation journey to enhance their quality of life.

Requirements

  • Licensed Physical Therapist
  • Experience in assessing physical condition and medical history
  • Ability to develop customized treatment plans
  • Proficiency in implementing exercises, stretches, and therapeutic techniques
  • Skill in educating patients on posture and movement techniques
  • Ability to monitor patient progress and adjust treatment plans
  • Experience working with other healthcare professionals
  • Proficiency in maintaining accurate patient records and documenting treatment progress
  • Knowledge of the latest techniques and advances in physical therapy

Responsibilities

  • Assess patient’s physical condition and medical history.
  • Develop customized treatment plans based on the assessment.
  • Implement exercises, stretches, and other therapeutic techniques to improve mobility and strength.
  • Educate patients on posture, movement techniques, and exercises for home care.
  • Monitor patient progress and adjust treatment plans as needed.
  • Work with other healthcare professionals (doctors, nurses, occupational therapists, etc.) to ensure comprehensive care.
  • Maintain accurate patient records and document treatment progress.
  • Stay up-to-date with the latest techniques and advances in physical therapy.
