About The Position

Immanuel Pathways PACE® is seeking a PRN Physical Therapist to support our team at 5755 Sorensen Parkway, Omaha, NE 68152. In this PRN role, you’ll provide therapy services to participants in our Immanuel Pathways Day Center. Mileage reimbursement is available at $0.725 per mile when travel is required. This PRN opportunity provides flexible, as-needed coverage support. Hours vary based on department needs and may not be available every month. When shifts are offered, they occur during weekday daytime hours and are usually completed by early afternoon. This role is ideal for someone looking to pick up occasional hours during planned PTO or unexpected coverage needs.

Requirements

  • Graduate of a physical therapy curriculum approved by one of the following: the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, the Council on Medical Education of the American Medical Association and the American Physical Therapy Association, or other equivalent organizations approved by the Secretary.
  • Two (2) years of appropriate experience as a physical therapist and have achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service.
  • One (1) year of management experience, preferably in a geriatric care setting required.
  • One (1) year of experience working with the frail or elderly population required, or completion of job specific training related to working with the elderly population must be completed within the first six months of hire.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Applicants must be currently authorized to work in the United States on a full-time basis.

Responsibilities

  • Provides physical therapy services to participants of the Immanuel Pathways.
  • Provides clinical physical therapy evaluation and treatment for program participants on acute, restorative or maintenance levels as needed by Participant Center, Nursing Home or in-home setting.
  • Integrates the physical therapy treatment plan into the Interdisciplinary Team plan of care.
  • Assists in the ordering, inventory, distribution and maintenance of durable medical equipment for participants.
