About The Position
LTC Therapy is seeking a Physical Therapist (PT) to join our rehabilitation team in Holyoke, MA. This role offers the opportunity to work in a skilled nursing setting focused on helping residents improve strength, mobility, balance, and overall functional independence.
Requirements
- Active Physical Therapist license required
- Strong communication and clinical reasoning skills
Nice To Haves
- Skilled nursing experience preferred but not required
Responsibilities
- Complete physical therapy evaluations and develop individualized treatment plans
- Provide therapy focused on gait training, transfers, strengthening, balance, endurance, and mobility goals
- Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to support resident care plans
- Monitor resident progress and update treatment approaches as appropriate
- Maintain accurate and timely documentation in accordance with facility standards
