About The Position

Focused Post Acute Care Partners (FPACP) is seeking a Licensed Physical Therapist to join their team at Focused Care at Mount Pleasant in Mount Pleasant, TX. FPACP is committed to delivering excellent long-term care and fostering a positive culture through its ROCKin recognition program, ROCKStar. The Physical Therapist will be responsible for evaluating patients, developing and implementing physical therapy treatment programs in collaboration with physicians, and administering various therapeutic exercises and modalities. They will also instruct patients and caregivers, and supervise Physical Therapy Assistants.

Requirements

Bachelor's Degree, Master's Degree, or Doctorate Degree in Physical Therapy from an accredited college or university.
Current license/registration by state of hire as a Physical Therapist.

Responsibilities

Evaluate patients and develop physical therapy treatment programs in collaboration with the physician.
Provide physical therapy treatments to patients.
Administer complex active and passive therapeutic exercises, therapeutic massages, and various modalities.
Instruct, motivate, and assist patients/caregivers in learning and improving activities of daily living.
Supervise and direct the efforts of the Physical Therapy Assistant, if one is assigned.
Establish, implement, and evaluate treatment programs, plans of care, goals, and discharge plans for patients related to Physical Therapy needs and in conjunction with the physician.
Revise plans and goals based on changes in patient status.
Provide direct patient care services to ensure the effectiveness of Physical Therapy programs.
Apply heat, light, water, and electrically based treatments/modalities (e.g., ultrasound, infra-red lamp, muscle stimulation, traction, hot and cold packs, paraffin baths, whirlpool, diathermy) to stimulate muscle/nerve function and/or for pain management, as prescribed, with proven competency and available equipment.
Prioritize, plan, treatment, and travel times to provide for maximum efficiency, yet meet the needs of the patients.
Ensure orders are carried out in a professional manner satisfying professional ethics, patient rights, and State and Federal codes and regulations.
Facilitate effective communication amongst other clinicians providing care to the patient.
Assist in coordinating the Physical Therapy program with other appropriate health care providers/disciplines providing services to the patient.
Participate in interdisciplinary patient care conferences as applicable and maintain ongoing communication with all disciplines involved in the patient Plan of Care.
