About The Position

FOX Rehabilitation is growing in Fort Collins, CO and is looking for passionate, licensed Physical Therapists to deliver exceptional one-on-one care to older adults in their homes or within Senior Living Communities. The role focuses on helping patients improve mobility, independence, and quality of life through the Geriatric House Calls™ model. This position is located in Senior Living Community partners, Greeley Place and Garden Square Westlake. It is ideal for Physical Therapists who value autonomy, flexibility, and meaningful patient outcomes, with the freedom to create and manage their own treatment schedule and build strong patient relationships. Clinicians may also incorporate telehealth and virtual care for greater flexibility and continuity of care. This is a high-impact PT role that allows you to positively impact the lives of the communities served.

Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
Degree from an accredited Physical Therapy program
Basic computer and EMR proficiency
Current CPR certification

Responsibilities

Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
Document effectively within FOX’s EMR system in accordance with regulatory standards
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
Provide telehealth / virtual care when appropriate to extend access and flexibility
