About The Position
Flagstar Rehab is seeking Physical Therapists (PT) for their Rehab Department in state-of-the-art Skilled Nursing Facilities (SNFs) throughout NYC and Long Island. PTs at Flagstar Rehab collaborate with a diverse, multi-disciplinary team to conduct evaluations, administer treatments, and provide patient/family education. The goal is to enhance patients' physicality and cognition, thereby improving their daily functional abilities. Flagstar Rehab offers employment options as a Direct Employee (W-2) or as an Independent Contractor (1099), with assistance provided for setting up a corporation free of charge.
Requirements
- Current NYS PT License and Registration
- CPR Certification
- EMR Competence
- Meets all State and Federal requirements for US employment in a Nursing Facility
- Malpractice/Liability Insurance (if applicable)
Responsibilities
- Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
- Supervise PTAs daily.
- Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
- Monitor and document patient progress with the use of innovative EMR software.
