About The Position

This position is for an Independent Contractor Physical Therapist to serve Goleta and the surrounding areas, delivering quality home healthcare to patients of all ages. FeldCare Connects aims to address the industry-wide challenge of finding care for homebound patients by connecting over 2,000 clinicians to patients in their community through contracts with over 900 home health agencies throughout CA, AZ, TX, and FL. By facilitating these connections and taking administrative burdens off agencies and clinicians, FeldCare Connects helps patients receive care faster, allowing clinicians to focus on treatment and resulting in better patient outcomes.

Requirements

• Ability and enthusiasm in working with all ages
• Current Physical Therapist license issued by the state board of Physical Therapy
• The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Completion of an accredited Physical Therapy program
• Reliable personal transportation
• Smartphone

Responsibilities

• Deliver quality home healthcare to patients of all ages
• Observe, record, and report the patient's response to treatment and changes to the patient's condition
• Determine the plan of care
