About The Position
As a Home Health Physical Therapist, you’ll provide skilled, one‑on‑one therapy services in the comfort of patients’ homes, helping them regain strength, mobility and independence. You’ll assess patient needs, develop individualized treatment plans, deliver evidence‑based interventions and educate patients and caregivers to support safe recovery at home. As a faith-based, not‑for‑profit agency, we are committed to caring for patients and families in the place they feel most comfortable—at home. Our services include skilled nursing, physical, occupational and speech therapy and medical social work.
Requirements
- Current licensure as a Physical Therapist in the State of Michigan
- Graduate of an accredited physical therapy program.
- At least one year of clinical experience
- Valid driver’s license and reliable transportation.
- Ability to work independently and manage time effectively.
