About The Position

The New Jewish Home Certified Home Health Agency (CHHA) provides home based care and rehabilitation therapy for patients recovering from an acute episode or other adverse health event. The New Jewish Home has long experience delivering rehabilitation and home care services. Our goal is to help each patient get well, stay well and return to health and independence as quickly as possible. The CHHA is currently seeking Fee for Service Physical Therapists to join their team!

Requirements

  • Must possess a current, unencumbered license to practice as a Physical Therapist in the State of New York.
  • Successful completion of Physical Therapy program at a school of Physical Therapy approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of American Medical Association or the committee on Allied Health Education and Accreditation of the American Medical Association.
  • Master’s degree or Doctorate degree in Physical Therapy.

Nice To Haves

  • One year of Physical Therapy experience preferred.

Responsibilities

  • Collaborates with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to develop and implement patient specific care and treatment plans.
  • Utilizes varied solutions to educate and promote the health and independence of each patient.
  • Assess, monitor, document and report progress of patients’ health and condition using required documents, reports and established timelines.
  • Provides supervision and evaluation of aides and physical therapy assistants.
  • Observes signs and symptoms of patients, reports to the physician and/or other appropriate staff as often as needed or as change in the patients’ condition requires.
  • Completes OASIS assessments timely, accurately and according to OASIS user’s manual definitions.
  • Completes all documentation, patient health status reports as required and forwards appropriately.
  • Supervises occupational therapy assistant according to state regulations and agency policy.
  • Assists with the coordination of the total plan of care and maintains continuity of patient care by liaising with other appropriate staff assigned to the same patients.
  • Participates in team/case conferences.
  • Initiates team/care conferences for complex and/or multi disciplinary patients when required to ensure coordination of care.
  • Performs other duties as assigned.
