About The Position

The Physical Therapist position is responsible for planning and administering physical therapy treatment plans designed to promote recovery from injury/illness and return to function improving overall fitness and health of patients of the Red Cliff Community Health Center.

Requirements

  • Minimum master’s degree, preferably doctorate in physical therapy from accredited physical therapy school.
  • Successful passing of National Physical Therapy Examination.
  • Must have active Wisconsin Physical Therapist licensure.
  • Must have a valid driver’s license and meet eligibility for tribal insurance.

Nice To Haves

  • Experience with pelvic floor therapy preferred.
  • Knowledge of and sensitivity for Ojibwe culture and traditions.

Responsibilities

  • Systematically assess, identify and initiate therapeutic management of injuries, chronic diseases, and illnesses within scope of practice.
  • Consult with and evaluate the patient to determine patient goals and needs.
  • Follows established protocols for therapeutic care including initiation of prior authorizations, accurate coding, and timely documentation of care provided.
  • Continuously evaluates plan of care, short and long term goals, regularly updating plans, including use of objective functional outcome measures, and determines appropriate discharge plan and date, and implementing home exercise programs according to the need of the client.
  • Work collaboratively with other providers including other therapists, specialty care, and the patient’s primary care provider.
  • Provide training/education to clients and caregivers as appropriate during and after completion of Physical Therapy services for acute, chronic, prevention, and supports healthy lifestyles.
  • Maintains equipment in safe, operational capacity and manages adequate stock of supplies.
  • Insures the confidentiality of all client specific information and data as required by the program and the Red Cliff Community Health Center.
  • Implement Quality Assurance measures in appropriate service areas.
  • Attend meetings, in-services, and community events as specified by immediate supervisor.
