About The Position

Legacy Healthcare Services is seeking an experienced, motivated, and compassionate Physical Therapist (PT) to join their team in Greensboro, NC. This hybrid role involves providing care at Heritage Greens and through the Better at Home program for home-based therapy across Guilford County. The Physical Therapist will work directly with senior adults in their residential community and homes, focusing on empowering them to lead lives with purpose, safety, and joy through private, functional-based, 1-on-1 treatment sessions. This role offers the autonomy to individualize care plans and build meaningful patient relationships.

Requirements

Degree in Physical Therapy from an accredited program.
Current Physical Therapy license in the state of North Carolina (or ability to obtain prior to start date).
Excellent interpersonal, clinical, and fiscal management skills.
Computer proficiency for electronic documentation and care planning.

Responsibilities

Evaluate, assess, and provide physical therapy services in accordance with physician plans of care and the State Physical Therapy Practice Act.
Deliver high-quality, 1-on-1 rehabilitation services both on-site at Heritage Greens and directly in patients' homes throughout Greensboro/Guilford County.
Evaluate home environments, mobility, gait, muscle strength, and ROM potential to build tailored rehabilitation programs.
Instruct patients, family members, and caregivers on therapeutic appliances, home exercises, and safe mobility strategies.
Participate in clinical conferences and coordinate care plans as part of a multi-disciplinary rehabilitation team.
Identify additional patient needs (such as PCA services) and make appropriate referrals.
Prepare timely progress summaries and supervise Physical Therapist Assistants (PTAs) in accordance with state regulations and company policy.
