Physical Therapist (Per-Diem) (Weekends Only)
About The Position
Emerge Nursing and Rehabilitation is hiring a per-diem Physical Therapist to work weekends. The role involves providing physical therapy in a long-term-care setting, specifically a skilled nursing facility. The therapist collaborates with other therapy teams, evaluates patients, documents care, and delivers treatments according to treatment plans and prescriptions. The therapist independently manages their workflow, including treatment time and electronic documentation, and aligns therapy plans with an understanding of PDPM and Medicaid and Medicare billing. Requirements
- New York State Physical Therapist license.
- Background or experience in long-term care, specifically a skilled nursing facility.
- Strong organizational, interpersonal, and communication skills.
- Ability to manage patients with different personality types and ability levels.
- Creative problem-solving skills.
Responsibilities
- Work collaboratively with all therapy teams.
- Complete evaluations and necessary documentation.
- Provide recommendations that support communications and assessments.
- Provide therapy as outlined in treatment plans and prescriptions.
- Offer treatments to improve patients’ inabilities and challenges.
- Independently manage workflow, including time for treatment and documentation of individual treatments and notes in the electronic records system.
- Align therapy treatments and plans with an understanding of PDPM and Medicaid and Medicare billing.
