Physical Therapist (PT) - Home Health - Full Time
About The Position
HomeFirst Home Healthcare is seeking a full-time Physical Therapist (PT) to join their clinical, home health field team. The company provides quality home health care to thousands of individuals in Tennessee and is growing. The mission at HomeFirst Home Healthcare is to provide compassionate home-based care, offering quality services to patients and their families while recognizing the valuable relationship with employees, providers, and the communities they serve. The PT will be responsible for conducting one-on-one patient care and patient/family teaching in the home setting, assessing needs, developing personalized treatment plans, and implementing therapeutic interventions to optimize functional abilities.
Requirements
- Graduate of an accredited physical therapy educational program.
- Current Physical Therapist / PT state license within the state of practice.
- Current CPR certification.
- Excellent organizational, time-management, written and verbal communication skills.
- Reliable transportation, valid driver’s license and current automobile.
Responsibilities
- Conducting one-on-one patient care in the home setting.
- Patient and family teaching in the home setting.
- Assessing patient needs.
- Developing personalized treatment plans.
- Implementing therapeutic interventions to optimize functional abilities.
