About The Position
Ultimate Therapy is seeking a Physical Therapist (PT) to join our skilled nursing rehabilitation team. We are looking for a therapist who is passionate about helping residents improve mobility, restore independence, and achieve meaningful recovery outcomes through individualized therapy care.
Requirements
- Active Physical Therapist license required
- Strong communication and clinical reasoning skills
Nice To Haves
- Skilled nursing experience preferred but not required
Responsibilities
- Complete physical therapy evaluations and develop customized treatment plans
- Provide therapy focused on gait training, strengthening, balance, endurance, mobility, and pain management
- Work with residents recovering from orthopedic, neurological, and medically complex conditions
- Collaborate with rehab staff, nursing, physicians, and families to coordinate resident care
- Monitor patient progress and maintain accurate clinical documentation
