About The Position

As a Physical Therapist – Rehab, your ability to design, plan, and implement personalized physical therapy intervention greatly enhances patient function and overall quality of life. This is a Per Diem position located at Golisano Rehab/Unity Hospital in Rochester, NY.

Requirements

  • Master’s Degree in Physical Therapy or Bachelor’s degree with prior experience as a licensed Physical Therapist required.
  • Current New York State license or a limited permit as a Registered Physical Therapist.
  • If a New York State license or limited permit is not held, must be hired within 90 days of graduation from a New York State or APTA - accredited physical therapist education program and meet Practice Exemption under New York State Physical Therapy License Requirements.
  • If working in Homecare or traveling between locations, an NYS license is required.
  • Developed leadership skills.
  • Patient-focused, with demonstrated care and compassion.
  • Exceptional communication and documentation skills.

Nice To Haves

  • Prior Physical Therapist experience preferred.

Responsibilities

  • Meet patient goals and needs by providing quality, evidence-based care; assess, develop, and implement an individualized treatment plan; help the patient accomplish the treatment plan and accept therapeutic devices by administering manual exercises; instruct, encourage, and assist patients in performing physical activities, such as non-manual exercises, ambulatory functional activities, and daily-living activities.
  • Assure continuation of therapeutic plan following discharge by designing home exercise programs; instruct patients, families and caregivers in home exercise programs; recommend and provide assistive equipment; recommend outpatient or home health follow-up programs.
  • Develop physical therapy staff by providing information; develop and conduct in-service training programs.
