About The Position

As a Physical Therapist – Rehab, your ability to design, plan, and implement personalized physical therapy interventions greatly enhances patient function and overall quality of life. Assesses and restores patients' function, alleviates pain, and prevents disabilities by designing, planning, and implementing medically prescribed physical therapy interventions. Requirements

• Master’s Degree in Physical Therapy or Bachelor’s degree with prior experience as a licensed Physical Therapist required.
• Current New York State license or a limited permit as a Registered Physical Therapist. If a New York State license or limited permit is not held, the applicant must be hired within 90 days of graduation from a New York State or APTA-accredited physical therapist education program and meet the Practice Exemption under New York State Physical Therapy License Requirements.
• Developed leadership skills
• Patient-focused, with demonstrated care and compassion
• Exceptional communication and documentation skills

Nice To Haves

• Prior Physical Therapist experience preferred.

Responsibilities

• Meet patient goals and needs by providing quality, evidence-based care; assess, develop, and implement an individualized treatment plan; help the patient accomplish the treatment plan and accept therapeutic devices by administering manual exercises; instruct, encourage, and assist patients in performing physical activities, such as non-manual exercises, ambulatory functional activities, and daily-living activities
• Assure continuation of therapeutic plan following discharge by designing home exercise programs; instruct patients, families, and caregivers in home exercise programs; recommend and provide assistive equipment; recommend outpatient or home health follow-up programs
• Develop physical therapy staff by providing information; develop and conduct in-service training programs
