About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Perris and the surrounding areas. FeldCare Connects is an app-based referral network that provides flexibility, independence, and strong per-visit compensation. They offer administrative support, one-on-one treatment, and an intuitive Electronic Medical Record system for easy documentation. The role involves observing, recording, and reporting patient responses to treatment and changes in condition, with the clinician determining the plan of care. FeldCare Connects connects over 2,000 clinicians to patients through contracting with home health agencies, aiming to provide essential care to homebound patients.

Requirements

• Current Physical Therapist license issued by the state board of Physical Therapy.
• The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Completion of an accredited Physical Therapy program.
• Ability and enthusiasm in working with all ages.
• Reliable personal transportation.
• Smartphone.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine the plan of care.
• Complete documentation using an intuitive Electronic Medical Record system, potentially during the visit.
