About The Position

Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout the DMV, including Washington, DC, Prince George's County, Montgomery County, Fairfax County, Loudoun County, Prince William County, Arlington, Alexandria, Falls Church, and surrounding communities. This PRN opportunity offers flexibility, professional autonomy, and the ability to make a meaningful impact across multiple communities. As a Home Health Physical Therapist, you'll provide personalized, one-on-one care that helps patients regain strength, improve mobility, restore balance, and rebuild confidence—all in the comfort of their own homes. You'll have the freedom to create individualized treatment plans, build meaningful relationships, and witness the difference your care makes every single day.

Requirements

Active Physical Therapist License in Washington, DC, Virginia, or Maryland (multi-state licensure is a plus)
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment, Communication, and Documentation Skills

Nice To Haves

Home Health Experience Preferred but Not Required
OASIS Experience Preferred
Experience with WellSky Home Health is a plus

Responsibilities

Complete physical therapy evaluations, Start of Care (SOC), reassessments, and discharge visits
Develop individualized plans of care based on patient goals and clinical needs
Provide skilled physical therapy services in patients' homes
Educate patients and caregivers on therapeutic exercises, mobility training, balance improvement, fall prevention, and home safety
Monitor patient progress and modify treatment plans as appropriate
Collaborate with physicians, nurses, social workers, and the interdisciplinary care team
Complete timely and accurate clinical documentation using WellSky
