The Physical Therapist (PT) is responsible for delivering high‑quality physical therapy services in accordance with the physician’s plan of care and all requirements outlined in the Personnel Services Agreement. Requirements
- Active CA Physical Therapy License
- Strong verbal and written communication skills
- Strong organization skills
- Possesses and maintains current CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
Responsibilities
- Provide skilled physical therapy services based on the written physician’s plan of care
- Conduct comprehensive, ongoing assessments and collaborate with the patient, caregiver, and interdisciplinary team to update and individualize the plan of care
- Educate and counsel patients, caregivers, and families on therapy goals, safety, and home exercises
- Prepare accurate and timely clinical documentation, including visit notes and progress summaries
- Communicate regularly with the physician and healthcare team regarding patient progress and changes in condition
- Initiate and implement the physical therapy program, instructing caregivers and appropriate staff on techniques they can safely perform
- Participate in case conferences to review goals, progress, and care plans
- Train patients in the proper use of prosthetics and assistive devices
- Identify needs for additional home health services and coordinate referrals when appropriate
- Participate in patient discharge planning and recommendations for continued care
- Provide in‑service education and training to agency staff as needed
- Engage in peer consultation and quality improvement processes
- Supervise Physical Therapist Assistants (PTAs) in accordance with state and organizational guidelines
