The Physical Therapist (PT) is responsible for delivering high‑quality physical therapy services in accordance with the physician’s plan of care and all requirements outlined in the Personnel Services Agreement. Requirements

  • Active CA Physical Therapy License
  • Strong verbal and written communication skills
  • Strong organization skills
  • Possesses and maintains current CPR certification
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order

Responsibilities

  • Provide skilled physical therapy services based on the written physician’s plan of care
  • Conduct comprehensive, ongoing assessments and collaborate with the patient, caregiver, and interdisciplinary team to update and individualize the plan of care
  • Educate and counsel patients, caregivers, and families on therapy goals, safety, and home exercises
  • Prepare accurate and timely clinical documentation, including visit notes and progress summaries
  • Communicate regularly with the physician and healthcare team regarding patient progress and changes in condition
  • Initiate and implement the physical therapy program, instructing caregivers and appropriate staff on techniques they can safely perform
  • Participate in case conferences to review goals, progress, and care plans
  • Train patients in the proper use of prosthetics and assistive devices
  • Identify needs for additional home health services and coordinate referrals when appropriate
  • Participate in patient discharge planning and recommendations for continued care
  • Provide in‑service education and training to agency staff as needed
  • Engage in peer consultation and quality improvement processes
  • Supervise Physical Therapist Assistants (PTAs) in accordance with state and organizational guidelines
