About The Position

Seeking Home Health Physical Therapists for the Somerset area Our compensation model provides a guaranteed annual salary in addition to a market-leading $ / productivity point rate: Fixed annual base salary: $70,980 / year (equivalent to a guaranteed 21 productivity points / week) $ / Productivity Point: $65 / point (for every point earned incremental to first 21 productivity points) About our Company Adaptive Home Health is a privately owned Home Health Agency delivering high-acuity, patient-centered care throughout Texas. Requirements

Current Texas Physical Therapist (PT) license
One year of physical therapy experience
Current Texas driver's license
CPR certification

Nice To Haves

Experience with Homecare Homebase is helpful, but not required
Previous Home Health experience is helpful, but not required

Responsibilities

Physical Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.
Evaluate patients and perform appropriate therapy interventions
Develop a comprehensive plan of care and coordinate multi-disciplinary care
Perform prompt clinical documentation and visit notes
Continued communication with physician, care team members, patient and family
Comfortable working in all aspects of the home health setting
Implement and maintain physical therapy protocols
Manage patient treatment and outcomes directly, or through PTA supervision
