About The Position

Sage Group HealthCare Staffing is seeking a Physical Therapist (PT) for a Home Health position in Everett, WA. This is a 13-week travel/contract position, and we are open to both local and traveler candidates. The role involves providing Physical Therapy services to home health patients, consulting with physicians and interdisciplinary teams, and developing treatment plans to restore function, relieve pain, and prevent disability. The therapist will also be responsible for driving to patient homes, completing charting, and ensuring patients and their families maintain dignity and independence.

Requirements

Physical Therapist: 1-2 years - Required
Recent/Current Home Health OT experience: 1 year within the last 3 years - Required
Home Care Home Base (HCHB) charting - Required
Must have valid Driver’s License and Personal Auto Insurance
PT License for State applying or Compact if applicable - Required
BLS Certification from AHA or American Red Cross - Required

Responsibilities

Provide Physical Therapy services for Home Health Patients
Consult with the physician and an interdisciplinary team in the development of a plan of treatment
Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential
Plans and prepares treatment plan based on evaluation of patient data
Provide in-home PT treatment programs to assist patients in restoring function, relieving pain, and preventing disability
Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home
Drive to patient homes in the assigned territories on a daily basis.
Complete charting and patient documentation in timely manner as per client policies and procedures
