About The Position

Empath Home Health, a member of Empath Health, is seeking a caring and motivated PRN Home Health Physical Therapist. The Home Health PT will make home visits in Hernando County (including Spring Hill, Weeki Wachee, Brooksville, and surrounding areas and the PT PRN will make 5-10+ home health visits per week. Requirements

Licensure as a physical therapist in the state of Florida
Valid FL driver's license
Graduate of an approved PT program with an active Florida license
Strong assessment, critical thinking, and patient education skills

Nice To Haves

Home health experience is a plus.
Prefers experience using Homecare Homebase (HCHB) EMR
Willingness to participate in weekend and holiday on-call rotations

Responsibilities

Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.
Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient’s ability to perform activities of daily living.
Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility.
Reports to the physician and designated agency representative patient’s reaction to treatment and any changes in the patient’s condition.
