About The Position

Lehigh Valley Health Network (LVHN) is seeking a Per Diem Physical Therapist (PT) for their Home Care team. This role involves evaluating, planning, directing, and administering physical therapy programs to help patients restore function, prevent disability, and achieve maximum independence. The Physical Therapist will assess patient conditions based on physician diagnoses, develop and update care plans in collaboration with patients and families, and improve quality of life through prescribed exercises, hands-on care, and education. The goal is to help individuals achieve fitness goals, regain or maintain independence, and lead active lives.

Requirements

  • Bachelor’s Degree Physical Therapy
  • Ability to work at various locations per diem.
  • Knowledge of best practices in physical therapy.
  • Demonstrates sound clinical reasoning in synthesizing data gathered in the evaluation and identifying treatment diagnoses.
  • Must be able to provide safe, compassionate, and competent physical therapy treatments.
  • Must be able to communicate with patients, family, and co-workers in a friendly, supportive manner.
  • Ability to travel to patient care assignments - Home Care
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
  • PT - Licensed Physical Therapist - State of Pennsylvania Upon Hire

Nice To Haves

  • Master’s Degree Physical Therapy or Doctorate Physical Therapy
  • 1 year clinical experience for Home Care

Responsibilities

  • Establishes and implements appropriate physical therapy plans of care to restore function, prevent disability, and help patients reach their maximum performance level.
  • Implements an individualized treatment plan through the use of skilled, evidence-based therapy techniques such as therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities.
  • Provides physical therapy treatment consistent with the patients' needs assessment.
  • Performs reassessment when indicated and modifies physical therapy plans accordingly.
  • Communicates patient progress and physical limitations to supervisor, physician(s), and other team members.
  • Implements a physical therapy plan of care to individuals based upon age-specific and population specific needs utilizing a developmental and patient/family directed approach.
  • Provides supervision in accordance with regulatory requirements, as appropriate.
  • Establishes a physical therapy treatment plan with measurable goals and time frames.
  • Provides cost effective care in accordance with departmental billing guidelines.
  • Completes and submits all documentation based on policies/standards and regulatory requirements.
