About The Position

Advanced Health Care of Colorado Springs is seeking a motivated and caring Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our team as an in-house therapist. We are a short-term rehabilitation facility focusing on the geriatric population with 34 beds. This part-time, PRN position involves PT evaluations and treatment, as well as supervision of in-house PTA staff. The Physical Therapist will play a crucial role in providing high-quality rehabilitation services to our residents, with a focus on restoring mobility, improving strength and function, and enhancing overall quality of life.

Requirements

  • Current state licensure or eligibility for licensure as a Physical Therapist (PT) or a Physical Therapist Assistant (PTA).
  • Ability to Commute: Colorado Springs, CO 80910 (Required)
  • Ability to Relocate: Colorado Springs, CO 80910: Relocate before starting work (Required)
  • Work Location: In person
  • Must have a Therapy License
  • Ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.

Nice To Haves

  • Minimum of 2 years Clinical experience in a skilled nursing facility or similar healthcare setting is preferred.
  • Preferred candidates to be vaccinated.

Responsibilities

  • Evaluate residents' physical abilities, functional status, and rehabilitation needs through comprehensive assessments.
  • Develop personalized treatment plans based on assessment findings, resident goals, and evidence-based practice guidelines.
  • Implement therapeutic interventions to address impairments, maximize functional independence, and promote safe mobility.
  • Provide one-on-one physical therapy sessions to residents, utilizing a variety of therapeutic techniques and modalities.
  • Monitor resident progress and adjust treatment plans as needed to optimize outcomes and achieve rehabilitation goals.
  • Collaborate with team members, including physicians, nurses, occupational therapists, and speech-language pathologists, to coordinate care and ensure continuity of services.
  • Educate residents and their families on exercise programs, safety techniques, and home exercise regimens to support ongoing rehabilitation and prevent re-injury.
  • Document all physical therapy services provided, including initial evaluations, progress notes, and discharge summaries, in accordance with facility policies and regulatory requirements.
  • Participate in resident care conferences, team meetings, and in-service training sessions to share expertise, contribute to care planning, and stay informed about best practices in rehabilitation.
  • Contribute to a positive and supportive work environment by demonstrating professionalism, teamwork, and effective communication skills.
  • Perform all other duties assigned.
  • Provide PT evaluations and treatment.
  • Supervise in-house PTA staff.
