About The Position
We are seeking a full-time Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our rehab team at a skilled nursing facility. Requirements
- Current PT license in Pennsylvania (for PT)
- Current PTA license in Pennsylvania (for PTA)
- SNF experience preferred (for PT)
- SNF experience preferred (for PTA)
Responsibilities
- Evaluate, treat, and educate patients according to physician orders
- Develop and implement individualized treatment plans
- Document patient progress in a timely and accurate manner
- Collaborate with interdisciplinary care team
- Maintain compliance with state and federal regulations
- Provide compassionate, patient-centered care in a SNF setting
