About The Position

Your Aging Therapist, LLC is seeking compassionate and dependable Physical Therapists and Physical Therapist Assistants who are passionate about helping adults enhance their mobility, safety, independence, and overall quality of life. This role involves working in both clinic and mobile/home-based settings, requiring travel to patients' homes and community locations.

Requirements

• Current Georgia PT/PTA license in good standing.
• Graduate of an accredited PT or PTA program.
• Current CPR/BLS certification.
• Valid driver’s license, reliable transportation, and appropriate auto insurance.
• Willingness to work in both clinic and mobile/home-based settings.
• Ability to travel to patients’ homes and community locations as needed.
• Strong communication, organization, and time-management skills.
• Ability to work independently while collaborating with the therapy team.
• Comfortable working with older adults and individuals with a variety of orthopedic, neurological, balance, and mobility conditions.
• Commitment to patient-centered, compassionate care.

Nice To Haves

• Licensure in both Georgia and South Carolina preferred, or willingness to obtain South Carolina licensure.
• Experience with geriatric or adult rehabilitation.
• Experience with home-based/mobile therapy.
• Experience with fall prevention, balance, neurological, or orthopedic rehabilitation.

Responsibilities

• Improve adult mobility, safety, independence, and quality of life.
• Provide therapy in clinic and mobile/home-based settings.
• Travel to patients' homes and community locations.
• Safely assist patients with transfers, gait, balance, exercises, and functional mobility.
• Complete patient documentation in a timely and accurate manner.
• Collaborate with the therapy team.
• Provide patient-centered, compassionate care.
