About The Position

Provide comprehensive rehabilitative care for a clinically diverse patient population, specializing in a wide cross section of neuro, ortho, and specialty areas like lymphedema and pelvic floor. Develop, implement, and regularly reassess individualized, patient-centered plans of care in collaboration with patients and families. Deliver evidence-based interventions across complex diagnoses, including neurological, oncological, orthopedic, cardiopulmonary, and medically complex conditions. Collaborate with the interdisciplinary team to coordinate care, optimize outcomes, and support safe, effective discharge planning.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Physical Therapy (PT) credentialed from the Illinois Department of Financial and Professional Regulation prior to hire date or job transfer date required. A valid license pending letter will be accepted until license is issued.
Bachelor's degree required.

Nice To Haves

Master's OR Doctorate degree preferred.
Prior Ortho outpatient experience.
